Make a Referral
Referral Overview:
The Aspen Group provides
homecare & hospice information for all referral sources. At this
time, Referrals can be completed by faxing the information or by calling
the information in to the corresponding Aspen Group office:
Aspen Home Health & Hospice
Blackfoot Home Health & Hospice
Rexburg Home Health & Hospice
Utah Home Health & Hospice
San Antonio Home Health
Greater Valley Hospice Alliance
Inpatient Referrals:
Medicare's section 204.2B of the HIM-11 states the attending physician
should write an order on the patient's hospital medical record
designating the type and frequency of home health services to be
provided. The physician should list the patient's home health agency of
choice prior to the patient's discharge. The hospital's discharge
planner, medical social worker or staff nurse will contact the chosen
home health agency to initiate the home healthcare.
Outpatient Referrals:
The patient has the ultimate choice in choosing a home health provider.
However, if a physician determines homecare is necessary for the
patient, any staff nurse in the institutional setting may forward The
Aspen Group relative patient information to begin the patient's homecare
admission process.
 Nurses generally will
provide patient evaluations within 24 hours of referrals if needed.
 The Aspen Group's
normal business hours are Monday through Friday from 9:00 am to 5:00 pm.
However, an on-call nurse is available 24 hours a day.

Donations & Volunteers
The Lori Olsen Foundation:
The Lori Olsen Foundation for Hospice Care is an organization dedicated
to helping The Aspen Group patients or family caregivers. Financial
contributions are used to help meet the patient's home medical needs.
Donations may be designated for either homecare or hospice patients.
How Will Donations Help:
A donation will be used to meet patients' needs, such as:
Medical equipment, including shower chairs, wheelchair ramps
and grab bars Occasional assistance with utility bills
Eyeglasses and hearing aids Food and special nutritional supplements
Medications
Before the funds are utilized, every effort is made
to locate other community resources to fund the need. Whatever the
donated amount, it is greatly needed and appreciated.
The Lori
Olsen Foundation is tax-exempt under Section 501(c)(3) of the Internal
Revenue Code. Gifts are tax deductible.
IRS regulations for
non-profit organizations require public disclosure of their request for
tax-exemption and their three most recent annual information returns. If

Volunteers are vital!
The
Aspen Group volunteers complete the circle of care that is extended to
each Aspen Group homecare and hospice patient. Volunteers improve
patients' quality of life by supplementing attention and companionship
provided by The Aspen Group medical staff. Additionally, Administrative
Support Volunteers assist with clerical support duties in all branch
locations.
